There is a mistake on my report. How
do I fix it?
If you are filing on
Paper
If it was a mistake in the original filing, you can file documents with
the Association to correct your account. Here are several examples:

The insured name was misspelled or incorrect on the declarations
page. File the endorsement that corrects the name just as you would
file any other endorsement (using a
Verification Slip with the original
and at least one complete copy).

The wrong Fire Marshal Tax was shown on the declarations page.
File the endorsement that corrects the Fire Marshal Tax. Since taxes
are usually based on the premium and the premium is not changing, show
'zero' on the Verification Slip but write in big letters "Fire Marshal
Tax Change Only." This way, we'll know to give this filing special treatment.

I filed a policy twice. Fill out a
correction form to reverse
the duplicate filing. File it as you would an endorsement
(using a Verification Slip with an original
correction form and at least one complete copy of the correction form).
These corrections will show up on your next monthly activity report.
If you are filing
Electronically
If it was a mistake in the original filing,
there are some instances where you can make filings on the
EFS to correct your account. Here are several examples:

I filed a policy twice.
For purposes of example, let's suppose the policy was for $1,000.
Locate the filing on the Your SLA Filings screen and click
Endorse. In the Effective Date field, fill in the
policy effective date. In the Premium field, put -1000 (negative
one-thousand). Click Submit. Now your SLA account will
balance with your records.

I filed a policy for the
wrong premium amount. For purposes of example, let's say you filed
a policy for $10,000 but it should have been for $1,000. Locate
the filing on the Your SLA Filings screen and click
Endorse. In the Effective Date field, fill in the
policy effective date. In the Premium field, put -9000 (negative
nine-thousand). Now your SLA Account will balance with your
records.
Alternatively, you could completely reverse out
the first filing (as discussed in the previous paragraph) and then
re-file it for the proper amount. This method is helpful to avoid
rounding differences in the taxes and stamping fees.

There are some instances
where you cannot make corrections using the EFS. Examples would
include incorrect Insured Name, Policy Number, Effective Date, Tax
Amount & Stamping Fee Amount. For these type of corrections,
contact the Association and speak
to the Help Desk.
If the Association Made
an Error
If you find an error that the Association made (unlikely though that
may be...), report it to us immediately and we will process a correction
internally that will show up on your next monthly activity report.
