Obtaining a Nonresident Illinois Surplus Line Producer
License
If you are an unlicensed nonresident you may want to

In order to obtain an nonresident Illinois surplus line producer license:
You must first have an
Nonresident Illinois
producer license. For information on obtaining the producer license,
click here, or contact the
Illinois Division of Insurance.
Please note that surplus line applications and
forms are not available for download on the
Division's website.
If you already hold a nonresident producer license, you may contact the Illinois Division of Insurance to obtain a
surplus line producer license application packet. Call Chris Lelys at (217) 782-5223 or Etta Mae
Credi at (217) 785-8155.
The application packet will include:
- Application Form
- Instructions for
downloading the SLA Membership Registration Package
- Biographical Affidavit
- Bond Form ($20,000)
- Certification Form
- Surplus Line Law (215 ILCS 5/445)
- Surplus Line Regulations (50 Ill. Admin. Code, Part 2801). Note that the
regulation has not been updated to reflect certain changes in the law. For
instance, the regulation still cites a surplus line tax rate of 3% when, in
fact, the rate was changed to 3.5%.
- List of Ineligible Surplus Line Insurers
Return the forms in the packet (except for the
SLA Membership Forms) to the Division of Insurance
at the address shown along with:
- Your license fee of $400 (payable to the Director of Insurance)
- Current (not more than 90 days old) Certification letter from your state of residence of your
license and surplus line authority
Return the SLA Membership Forms to the
Surplus Line Association of Illinois. Membership in the Surplus
Line Association is free and continues as long as your license is valid.
Surplus line licenses expire one year from date of issue.
