Obtaining a Resident Illinois Surplus Line Producer
License
If you do not hold a surplus line producer license, you may want to
first review our Summary of
Filing Procedures & Surplus Line Rules in Illinois.
In order to obtain an Illinois surplus line producer license:
You must first have an Illinois
producer license. For information on obtaining the producer license,
Illinois Department of Insurance.
Please note that surplus line applications and
forms are not available for download on the
Department's website.
If you already hold a producer license, you may contact the Illinois Department of Insurance to obtain a
surplus line producer license application packet. Call Johnel Buter at (217) 782-5223 or Etta Mae
Credi at (217) 785-8155.
The application packet will include:
- Application Form
- Instructions for
downloading the SLA Membership Registration Package
- Biographical Affidavit
- Bond Form ($20,000)
- Certification Form
- Surplus Line Law (215 ILCS 5/445)
- Surplus Line Regulations (50 Ill. Admin. Code, Part 2801). Note that the
regulation has not been updated to reflect certain changes in the law. For
instance, the regulation still cites a surplus line tax rate of 3% when, in
fact, the rate was changed to 3.5%.
